Currently it is a for profit system for the insurance companies. They insure the healthy individuals playing the odds that those individuals will not get sick or injured. If you have a pre-existing condition no one will give you coverage, your only option is to use the emergency room. Most individuals in the U.S get their coverage through an employer because the premiums are cheaper.

Technically, there’s no US healthcare system per se. Some people get health insurance through employment if the employers offer it. (That’s up to the employers to decide). Others can buy health insurance on their own (but, most can’t afford it). Then there’s Medicaid or Medicare for certain indigent and elderly people. That is handled by some form of government (but, not very well, if you ask most people). An HMO is an insurance that requires you to get treatment in their own network. You can get that through an employer or paying for it yourself, but again, the cost is often prohibitive.
