What is required to have the expiration date on an existing certificate of inspection (operating permit) extended?
680 IAC 2-3-9(a)(2) provides that the existing certificate may be extended for a maximum period not to exceed six (6) months. The requirements for approval of such extension are as follows: • A written request from the owner or user for such extension in the form of a letter that states the operational difficulties that necessitate the request. • Written approval from the inspector or the inspection agency having responsibility for inspection of the equipment. It is recommended that the approval and the request be submitted together in order to minimize the chances of their separation from one another.
