You need to have your step-3 scores in April when the program starts paper work after you match and then if the annual cap is not over you might be able to start your residency on time (that is July or August) but if the annual cap is reached then you will file your paper work and the H-1 visa status would be issued/started in October when the annual cap starts for next year.
