What is Continuum?
The UVa Health System has its own home health agency, Continuum Home Health Care (CHHC), which provides healthcare for patients who require care at home. The services are provided by qualified nurses, pharmacists, therapists, social workers, nutritionists, and aides. From its own on-site home infusion pharmacy, Continuum can provide prescribed medications, nutritional products, and home infusion equipment if needed by patients to complete their care at home. Continuum is under the operational direction of a clinical care services administrator and carries out its day-to-day functions under the direction of an Agency Director. The clinical governing board of the University of Virginia Medical Center, the Clinical Staff Executive Committee, provides clinical oversight.
