What is case management?
Case management is provided by Case Coordination Units in Illinois. Case Managers conduct home visits and provide assessments of an individuals needs and for what services s/he may qualify. Then Case Managers explain the options available for clients and their families to consider and establish recommended plans of care. Knowing all the options helps seniors and their families make decisions on what best will meet the needs. The Health Advantage case management program is a personalized, multidisciplinary process to assist patients and family members of patients who face catastrophic illnesses and long-term recoveries in meeting health needs. Case management promotes individual health-care management while facilitating appropriate health-care measures within the most cost-effective environment. Your physician will direct all care if you become part of the case management program. Registered nurses who are case managers will follow your care and cost of benefits, and be available to answer questions. Examples of situations in which case management may assist in conservation of limited benefits include, but are not limited to, emergency hospital admissions; rehabilitation services; home-health care, including home IV; pain management; hospice; and transplant-related services.
Case management is a service available to all PacificSource members who have complex or chronic medical conditions and require support to manage their healthcare needs. It is a service aimed at improving health outcomes and quality of life and reducing healthcare costs. Our case managers are registered nurses with extensive clinical experience. They work collaboratively with you and your healthcare providers to provide improved clinical, humanistic, and financial outcomes for you. Case management is a program that allows for early intervention and management of cases involving serious or catastrophic injuries or illnesses. Case managers are registered nurses who work with the patient, the providers and the claim administrator to ensure that the patient is receiving the right level of care at the right time. Case managers help to make the most efficient use of health plan benefits over the course of long-term treatment.
