What happens during the initial visit?
Most of the time, youll be talking with Dr. Glassman. Everything you discuss remains between the two of you. After your extensive conversation, hell ask you to relax in the waiting room for about 15 minutes. During this time, Dr. Glassman will record a CD specific to you and the reason you came to see him. When hes finished recording the CD, hell hand it to you along with a Compliance Sheet. The Compliance Sheet will contain very important information regarding the proper use of your CD. Following the instructions on the Compliance Sheet is vital to achieving optimal results.
Prior to your initial visit, please prepare a brief chronology of your neurological history outlining your symptoms, when they began, how they have changed, tests you have had, medications you have received for treatment and their effect, medications you are currently taking for any reason, with the dose and any non prescription or alternative medications you are taking. Please bring a copy of the results of any testing you have had performed for your neurological problem and copies of all MRI pictures or the disc with the images. For your initial visit, please arrive 30 minutes ahead of your scheduled appointment time to complete the necessary paperwork. You will be asked to provide some patient demographic and insurance information and to complete a medical history form. This information will enable the doctor and staff to better serve you. During the initial visit your architect will spend time asking you questions and finding out what you want to achieve. They will discuss the options that you have available along with the feasibility and implications of these options. They will then take measurements of your property and sketch out a plan of your property as it currently exists. Once this is completed your architect is now in a position to draw up initial design sketches to help you visualise your design and provide input. These drawings will be yours to keep so that you can refer back to them at a later date. The initial visit is designed to be an interactive session so you will be heavily involved in the process providing information about your lifestyle, your ideas and your feedback on proposed solutions.
