If company is ICC Regulated, you must attach a copy of your federal safety ratings. You may obtain the Safety Rating from the Federal Motor Carrier Safety Administration at (800) 832-5660. If a Corporation or Limited Liability Company, you must attach a copy of your certificate of good standing from the state of incorporation. If Kentucky based, you may obtain the certificate from the Secretary of State’s office at (502) 564-2848.
