In order for the referring agency to obtain documents from a clients file what is needed?
Please remember services are voluntary. In order to obtain any documentation there must be a release of consent signed by the legal guardian if the client is not 18 years of age. It is the agencys policy to not release official progress notes. This is privileged communication and an official Medicaid document. Weekly or Bi-weekly progress logs can be submitted with a release from the legal guardian or client if 18 years of age.
