There is no ID card required to prove eligibility or receive benefits under MetLife plans. Just notify your dentist that you participate in MetLifes PDP. Your dentist can easily verify information about your coverage through a toll-free number. For your convenience, a MetLife dental reference card with MetLife contact information will be provided to you after you enroll for the plan.
