A properly completed Form 130-U, Application for Texas Certificate of Title c. A Form VTR-61, Rebuilt Affidavit recording component parts used (and their origin) d. Safety Inspection Form e. Proof of Insurance f. A $65 rebuilt salvage fee g. $33 Title Application Fee for new negotiable Title h. Registration renewal fees will be assessed (if applicable). Upon completion of Step 3, the vehicle title will be appropriately branded and the vehicle may again be operated on public roadways.
