DENIED LINE ITEMS are identified as follows: (1) Line Processing Indicator Code is equal to ‘B’, ‘C’, ‘D’, ‘I’, ‘L’, ‘M’, ‘N’, ‘O’, ‘P’, ‘T’, ‘U’, ‘V’, ‘X’, ‘Y’, ‘Z’ (2) Line Processing Indicator Code is equal to ‘R’ or ‘S’ and the Line Allowed Charge Amount are equal to 0 .
