How do I enroll in the Capdent NY Plan?
First, review the list of participating dentists and select a provider that is conveniently located for you and your family (all family members must use the same dentist). Then go to “sign up,” complete the enrollment form and enter the name of the dentist you chose. On the same secure from, you will be prompted for credit card or check information, whichever is more convenient for you. Please click here to sign up.
