All students born after 1956 must prove their immunity to measles (Rubeola). You may prove this immunity in one of two ways: 1. Documentation signed by a health official of the dates (month, day and year) of 2 measles vaccinations, at least one of which was given in 1980 or later. The first dose must have been given on or after the first birthday. Please try to complete your vaccinations at least 30 days prior to your arrival at ISU. Please ask your physician to complete the immunization requirement form found at: http://www.health.iastate.edu/forms/ 2. A physician-verified history of measles disease. Students unable to demonstrate their immunity to measles must receive 2 immunizations against the disease. Exemptions from the measles immunization requirement may be granted for certain health conditions documented by a health official; or on religious grounds verified by a religious official.
