I find, in general, that speculating on the perceptive state of a dead pilot and stating that as fact is inappropriate. Clearly, there may be reasons for speculating this as the cause of an event, and they should be listed as Findings. But to specify it as Probable Cause needs to be very carefully framed, which it usually is not. So I try not to feed the “speculation” monster. For this reason, there are no keywords which focus on pilot mental state, such as spatial disorientation or “get-there-itis.” These are VERY well-established and very real and very dangerous phenomena and are of concern to any pilot. They just don’t have a place here.
