By filling out and submitting your Organ Donor Registry form, you are entered into a secure, computerized registry via your Personal Health Number (CareCard). If in the future, you are declared dead in hospital, the medical staff that are with you and your family will enter your Personal Health Number into the system. Healthcare providers can then print your registration and show to family members indicating your decision.
