Will the Mayo Clinic Tobacco Quitline or Mail Distribution Center inform my physician about the NRT being sent to me?
No. If you are an active duty member, it is your responsibility to inform your health care provider of the medications you are taking. To ease this transfer of information, you will receive a Counseling Encounter Summary (CES) which will provide a brief description of your smoking history and the medications that were recommended and provided to you by the Mayo Clinic Mail Order Distribution Center. You are responsible for sharing this information with your health care provider.
