Will Patient Navigation for Cancer Patients and similar direct services be considered as an implementation project to reduce health disparities and achieve health equity?
The CTG initiative is designed to create healthier communities by 1) building capacity to implement broad evidence- and practice-based policy, environmental, programmatic, and infrastructure changes in large counties, states, tribes, territories, and rural and frontier areas; and 2) supporting such interventions in five strategic areas. Patient navigation would be a mechanism to support the strategic direction of increased use of high quality clinical preventive services. However, funding for direct services is not within the scope of this announcement and will not be available through the CTG FOA.
