Coverage is provided for one routine eye exam per year only when the provider is in the Blue Options network. Routine eye exams will not be covered out-of-network. Members must let the provider know they are a Blue Options member when they arrive for a visit. This plan does not provide coverage for the fitting of contact lenses, glasses or other hardware.
