Why would the Health Advocates, Disease Management Nurses and exchange clinical information?
If you have a disease such as diabetes, a leading cause of adult blindness, may recommend to your provider that they conduct additional eye exams with you to monitor for complications. For example, when your provider contacts to pre-certify that you have vision coverage, may advise your provider to conduct further exams as part of the comprehensive vision exam. While will advise your provider of the exams to conduct, they will not share the medical reasons for the exams. No private health information will be shared with providers. If your provider detects any medical condition through the eye examination, the provider will refer you to your primary care physician. However, if the provider submits the clinical data, will provide the Health Advocates with that clinical data to enable the nurses to provide you with appropriate outreach and support for your condition should you choose to use it.
