The PDF Print version of the survey has been provided for your convenience to help you collect data before completing the survey and provide a record of your responses in the event that you should lose the cached data submitted on the online survey. Please note that the ACRL survey is an online survey only and that the printable forms are merely provided to help you collect data. Do not mail or fax your survey responses to CIRSS. Problems in submitting ACRL survey data online should be reported to CIRSS by phone (217-333-1980) or email (surveys@lrcmail.lis.uiuc.edu).
