Why should patients with diabetes be enrolled in the Maryland P3 Program?
• The Maryland P3 Program gives patients the opportunity to meet with a specially trained pharmacist to receive individual care focusing on patient education and self-management training. During the sessions pharmacists also focus on patient education related to foot, skin, and eye care, blood pressure and weight management. Pharmacists will work collaboratively with the patient’s physician and other health care providers. • It has been reported that patients who actively participate in chronic disease self-management programs have improved clinical outcomes, increased quality of life and satisfaction with pharmacy services, and reduce health resource utilization and cost. • Participating groups incentivize their members to enroll in the program by waiving the copay for medications to treat the patient’s diabetes and by offering gift certificates to their pharmacy.
