It is imperative that this section of the form be completely filled in. This information is used to determine whether or not the specimen meets our criteria for testing. Failure to provide this information may result in delayed testing or rejection of the specimen. The “Signs/Symptoms” and “Pregnancy Status” boxes must be checked even though patients were seen in Prenatal or STD Clinics.
