Why Mazda b3000 clutch fail?

It is pretty hard to such a question without any more info. How many miles, driving conditions, fluids changed regularly. Often times clutches fail for a multitude of reasons but most often it is from driver abuse. By this I mean proper care for the clutch and trans. If it happened all of a sudden and you take care of it regularly, I would say that something inside the clutch pack went bad. Could be the throw out bearing but likely the springs that are on the inside of the clutch pack that keep the drivshaft from spinning in nuetral are weakend or broken. However, my initial guess would be a throw out bearing but like i said it difficult to diagnose w/o more info.
