Why is the Maryland P3 Program only offered in Allegany County?
• The county has a significant number of patients with diabetes (5000), and local payers, pharmacists, and other leaders have come together to organize and implement this program. • The intention is to spread the program to other areas of the state where there is interest, and to other chronic diseases such as asthma, dyslipidemia, hypertension and depression.
