Why is the date in block 42 of the DA Form 61 so important?
Date is used to determine if you need an Active Federal Service (AFS) waiver (12 or more years of AFS). Page three of the DA Form 61 cannot be updated until after you have been NC-NS. Then you will require a new DA Form 61 for reapplication per the web site examples. If you need to update you APFT, you can send the copy of the latest APFT scorecard and it will be added.
