PHC will be converting the Advocate Application to an online process in the near future. The Application Form has been revised a number of times as the company has progressed through the pre-launch phase. PHC wants to ensure the Application contains all information needed and that it is in the best format before the Application is sent to a computer programmer to convert it for use online.
