WHY HAS INFORMATION ON THE PATIENT’S WHITE ID BAND BEEN REMOVED?

Proper patient identification is a serious patient safety concern. Unnecessary information has been removed from the ID band sticker for two reasons. 1). To provide only the consistent identification data needed to properly identify a patient and 2). To significantly reduce the number of times that patient’s ID bands need to be changed. Each unnecessary change provides an opportunity for error. Unnecessary items eliminated from the ID band (but always available in Meditech): room number, sex, age, service, marital status, admitting doctor, attending doctor, admission date/time.
