Why, for me and a sibling, does Novacaine wear off before the procedure even starts?

Thank you. what can I do about bad breath that come for the throat area and little smelly yellowish balls that comes up to my mouth. This situation I have is very very stressful. My four year old grandson had a fall which split his lip and may have loosened one of his upper teeth. We took him to the e.r. where they glued his lip together. He did complain of his tooth hurting after the fall. About a week or so later, we noticed one of his upper front teeth is turning brown. Since these are baby teeth, is there anything that needs to be done?
