pathogens need an environment like a to body to grow. Extremophiles have evolved in hot temps, extreme alkalinity etc- conditions not usually found in the body. They will therefore either die, or be outcompeted in our bodies by bacteria that have evolved to live in those conditions. All of our bodies contain oxygen in some form, so anaerobic extremophiles will not do so well in us, as aerobic bacteria reproduce and grow faster.
