I do not know about either pharmaceutical or Iran, but from I can speak from my personal experience. In my country I changed a few jobs and industry, always with success so I expected that moving to another country would not make much of a difference: I had the skills. However, my destination country was much more different from my country of origin in terms of culture and values than I would expect, so I realised that it took me a while to understand (this is mainly in terms of unwritten rules and human relationships). And to operate effectively in an environment, especially at a senior level, you need to understand in quite a good detail the culture, context, habits, language and non-verbals to a quite detailed level. So I suspect that cultural difference is a subject you might want to investigate.
