Why does OSP Require a Form 10?
The Form 10 is the internal academic approval form for sponsored programs. The Form should be completed and submitted with all proposals. Effective March 11, 2009, OSP will accept a signed scanned Form 10 and does not need the original paper document. The current version is dated February 6, 2009.
