Why does one CORI request form have a signature line and the other request form doesn’t?

Because CORI checks are mandated by law for volunteers, the organization is not required to obtain the volunteer’s signature in order to complete a CORI check. However, you may wish to inform the volunteer that a CORI check will be conducted prior to being accepted as a volunteer. CORI checks for employees are discretionary and are not mandated by statute. Therefore, you will need to obtain an employee’s written consent prior to completing a CORI check on the employee.
