To provide ample time for all Motorola employees to participate in the Health Risk Questionnaire, it was decided to provide a second period for HRQ completion. The second period will be from 22 September through 5 December 2010 and again 1 January 2011 through 21 January until midnight Central Time. Based on web development timelines, some sections of the HRQ website might not reflect this change, but the second period deadline is 21 January 2011, not 15 September 2010.
