This was actually answered in one of the past episodes. Dr. House had a horrible infection in his leg- so horrible they wanted to amputate the leg but he refused – choosing to die comfortably instead. Once he’d gone unconscious from the meds; however, his power of attorney person authorized for a procedure to be done on his leg-cutting out a great deal of tissue and muscle- leaving him with a limp and a 50% chance of living. Well, he lived and has the limp.

His constant leg pain also answers why he’s always after methods of relieving pain through painkillers or drug treatments in the show.

He walks with a limp because of an infarction (death of a macroscopic area of tissue in an organ due to loss of adequate blood supply) in his leg muscles.
