The Registration Form was created by Per-Se to set you up to send claims through us electronically. Enrollment Forms are created by payers for setting up their own systems to accept the electronic claims sent to them through Per-Se. The Per-Se Client Registration Form is the only paperwork required for sending claims to most commercial payers through Per-Se. However, Medicare, Medicaid, CHAMPUS, and some Blue Cross/Blue Shield payers require providers to complete enrollment forms before they will accept claims sent electronically. Even if you are already sending electronically to such payers, you may have to complete new enrollment forms before you can start transmitting claims to them through Per-Se. All of the Enrollment Forms you requested, each with its own set of instructions, were included in your initial enrollment packet. Should you wish to send claims to other payers, your Client Enrollment specialist will mail you any necessary paperwork upon your request.
