Why didn’t Medicare pay?
Most Medicare plans have an out of pocket deductible that you are required to pay before Medicare begins to pay. Some other out of pocket expenses includes co-payments and coinsurance. Please contact your Medicare plan if you are uncertain of your deductible and/or other out of pocket expenses associated with your plan. In addition, you may have received services from a provider that is not in the network of your Medicare HMO. If so, you will be responsible for paying the out-of-network balance of the physician charges.
