Below are some of the most common reasons this may occur: • The laboratory is a new start up lab and should have been enrolled in a PT program, but was not for that event. This acts as a trigger for the lab to get enrolled and authorize access of the PT scores to COLA. • The laboratory has changed PT providers and forgot to authorize the release of the PT scores to COLA. A PT authorization form is included with each letter. The laboratory should complete the form, making sure they include their CLIA number, COLA ID, and PT provider account number. • The laboratory has been in existence for some time, but forgot to enroll in a PT program for the next year. PT enrollment covers one calendar year. During the months of September and October, each PT provider sends out their renewal notices. It is important that the laboratory renews with their curre
