Why Chronic Condition Self-Management?
Internationally, and particularly in developed countries, the disease burden is shifting rapidly from acute to chronic conditions. Health Care practices must now change in response to this challenge. Chronic Condition Self-Management (CCSM) is widely recognised as a key strategy which needs to be included in health care practice. The Flinders Program of Chronic Condition Self-Management support is a generic approach which can be used across a wide range of chronic conditions. It enables the clinician to develop skills in: • collaborative problem definition; • goal setting; • planning; • motivational techniques; and • outcome measurement.
