Why can’t I create a Grade Book column using the Grade Book Options?

When creating a Grade Book column through the Grade Book Options which contains the same name as another Grade Book column, no error appears; but the Grade Book column does not appear. Review the Grade Book columns to ensure that new columns are not created with the same names as existing columns.
