Why are PROMISE’s palliative care consult numbers different from transformational measure numbers?

The PROMISE center only counts a palliative care consult if the note is titled with the words ‘hospice’ or ‘palliative’ in the last month of the Veteran’s life. Some facilities label a palliative care consult with other terms. The PROMISE Center does not count those other terms as consults nor does the Center use the transformational measure numbers.
