Why are background check authorization forms returned unprocessed?
Below are the most common reasons an authorization form is returned unprocessed: • The faxed form cannot be read. • Required information cannot be read or is incomplete. • An outdated background authorization form is submitted (To obtain a current copy of the form, you must go to the Internet site provided above in #1). • The date of the signature is older than three (3) months. • The date signed by the authorized DSHS staff is older than three (3) months. NOTE: To ensure forms are not rejected, the authorized DSHS staff should review the form for completeness and legibility prior to sending or faxing the form to BCCU.
