Please be sure your enrollment fee has been processed and/or your unity ID and password are correct. If a password reset is needed, contact the NCSU Help Desk at 919515.HELP. Records must be legible and include your name and date of birth on each page, the name of the official source providing the records (i.e. a doctor’s office or school), and must be in English at the time of submission.
