Who will be involved in providing hospice services?
Hospice services are coordinated and delivered by a team of healthcare professionals of various disciplines, including physicians, registered nurses, social workers, home health aides, spiritual counselors, therapists, dieticians and volunteers. This team is known as the “interdisciplinary team.” The interdisciplinary team develops a plan of care which outlines the care the team perceives to be necessary in order to meet the needs of the patient and family. The plan of care is developed in order to manage the patients pain and symptoms as well as meet the patients psychological, social and spiritual needs.
