Who should sign part 4 of the application if I do not currently have a seminary teacher?

If you attended seminary and have not yet graduated from high school, you must print the instruction sheet and give it to your seminary teacher. If you did not attend seminary or you have already graduated from high school, you will have the option to fill out a self report.
