The Certificates of Correction form (PTO/SB/44, same as PTO-1050) may be obtained by contacting the General Services Division at 1.800.786.9199 (1.800.PTO.9199) or 571.272.1000. Also, the forms are available on the USPTO Internet site at: http://www.uspto.gov/web/forms/index.html.
