Who should I list as the vendor for Appendix E: Conflict of Interest Certification?
The Conflict of Interest Statement should be completed by the applicant with respect to commercial vendors and developers of technologies or services intended for healthcare practice. The COI form addresses whether the lead applicant has a conflict with a potential vendor. All applicants should fill it out per the COI definition from the procurement regulations below. “Such a conflict would arise when the employee, officer, or agent, or any member of his or her immediate family, his or her partner, or an organization which employs or is about to employ any of the parties indicated herein, has a financial or other interest in the firm selected for an award.
