Who should have a home language survey?

There must be a home language survey in each students cumulative file. The home language must be completed for each student new to the district and for students previously enrolled who do not have a home language survey in their cumulative file. Only one home language survey should be completed for each student. It is not necessary to complete one annually. The home language survey must be signed by the students parent/guardian for students in grades PK-8. Students in grades 9-12 may sign their own home language survey.
