While either a licensed doctor or health care professional or a qualified employee acting at his or her direction (such as a nurse) may administer the dose of GARDASIL, the “Health Care Professional Certification” section of the rebate request form can only be signed by the licensed doctor or health care professional (such as a nurse practitioner or physician assistant). This section cannot be signed by a registered nurse. This section cannot be filled out by the patient or the person responsible for out-of-pocket cost for the dose of GARDASIL.
