While either a licensed doctor or health care professional or a qualified employee acting at his or her direction (such as a nurse) may administer the dose of GARDASIL, the “Health Care Professional Certification” section of the rebate form can only be signed by the licensed doctor or health care professional (such as a nurse practitioner or physician assistant). This section cannot be signed by a registered nurse. This section cannot be filled out by the patient or the person responsible for out-of-pocket cost for the dose of GARDASIL.
