Who should be involved in completing the assessment?
The best way to complete the PHC assessment is through team effort. Team members might include faith-based organization leaders (such as the pastor), staff or congregation members dedicated to improving the health of the faith community (such as a parish nurse), interested congregation members, and relevant representatives from the surrounding community. In addition to making assessment completion more efficient and effective, a team approach encourages ongoing collaboration for improving the health of congregation members.
